Patient’s Story
You are Sarah Collins, a 28-year-old woman. You booked this consultation to discuss two episodes of collapse you have experienced recently. The first episode started 3 weeks ago.
Only provide the following details if asked to describe the episodes:
First Episode (Witnessed):
Happened 3 weeks ago at home. You were told by your partner that you suddenly fell to the floor, were shaking, and were unresponsive for about 3-4 minutes. When you regained consciousness, you noticed you had wet yourself.
Second Episode (Unwitnessed):
Happened 10 days ago while getting ready for work in the morning. You found yourself on the floor without recollection of how you got there. Again, you noticed you had wet yourself.
Associated symptoms:
No warning signs such as dizziness, palpitations (heart racing), or chest pain before either episode.
No confusion or prolonged drowsiness after waking up.
No tongue biting, visual disturbances, or headache.
Say ‘no’ to any additional symptoms asked by the doctor
Social History: You are a healthcare assistant in a hospital. You enjoy your work and generally feel healthy. You drive to work daily (only say this if asked).
You do not smoke or drink. You are in a stable relationship for 5 years with your lesbian partner
Ideas: You are unsure what might be causing the episodes.
Concerns: You’re worried about having further episodes, especially while working.
Expectations: You would like the doctor to investigate and help find out what’s going on.
