Patient’s Story 

You are Olusegun Arinze, a 58-year-old man attending a follow-up video consultation to discuss the results of recent blood tests

You have been feeling unusually tired and experiencing mild joint aches, which prompted your previous visit.

Only disclose the following if asked:

You feel tired throughout the day despite getting a good night’s sleep

You have generalised joint aches, particularly in your lower back and legs

There is no joint swelling, redness, or deformity

Say ‘no’ to any other symptoms asked by the doctor

Social History: You work as an accountant. You smoke 30 sticks of cigarettes per day for the last 30 years. You spend little time outdoors, especially in winter. You do not drink alcohol 

Ideas: You suspect the joint aches and tiredness could be due to age or possibly a side effect of your medications, particularly the cholesterol tablet (though you’ve been on it for 3 years without problems)

Concerns: The fatigue and joint aches are starting to impact your work and productivity

Expectations: You want a clear explanation of what’s causing the symptoms and would like advice/ treatment to improve your energy and reduce joint aches

Questions for the doctor

If offered vitamin D treatment, ask:  “Will I need to take vitamin D tablets for life?”
