You are Amir Khan, attending today to discuss your recent blood test results. 

You believe your HbA1c level is likely to be high. This is because you’ve not started insulin despite previous recommendations because you dislike needles, and you’re also concerned it may impact your work. 

You’re a truck driver and fear losing your license if you’re started on insulin.

You’ve noticed you’re frequently thirsty and passing urine more than usual — you understand this is linked to your poorly controlled diabetes.  

You’re taking all your other prescribed medications regularly. Say no to any other symptoms asked. You’re open to the doctor’s suggestions — but make it clear you really do not want an injection.  

Social history: You don’t smoke or drink alcohol, but your diet isn’t ideal — your meals are often high in carbohydrates, mostly home-cooked by your wife.

Ideas: You suspect your high sugar levels are worsening your symptoms.

Concerns: You’re worried about complications — your uncle lost a leg due to diabetes, and you fear something similar.
